Our Best Practice Referral Process - 5 quick steps:
  1. We will take basic details verbally, discuss current vacancies and placement cost and an initial matching indication given prior to completing a Referral Form.
  2. We will ask you to supply any relevant reports or other documents by fax or email.
  3. We will then need you to provide 'in principle' confirmation of funding approval and Referral Form completed.
  4. We will then arrange an assessment visit to meet the young person, their current carers and - where possible - the placing Social Worker.
  5. We will make the decision on the offer of a placement.
Additional information that will be required (if applicable) includes:
  • Confirmation of funding agreement.
  • A copy of a Core Assessment (if available) or any other assessment completed.
  • An immunisation record.
  • Details of the last Health Assessment.
  • A copy of any Statement of Educational Needs.
  • A copy of any assessment carried out by the Psychiatrist involved in the prescription of any current medication and details of when the medication was prescribed.
  • Full details of any criminal offences committed by the young person.

All contractual documentation will be appropriately completed upon receipt of the current Ofsted Inspection Report, Statement of Purpose for the placement and Insurance Details can be accessed via West Midlands Placement Database CCSR Individual Placement tendering system or alternatively should you not have access to these databases, these can be emailed /faxed.

Where appropriate the placing Social Worker, parents and the young person may visit an identified Clifford House resource and an education facility prior to the placement being accepted by the Placing Authority.
